This Disclaimer applies to the Inferno FMEA website, application, software platform, AI-assisted tools, reports, exports, documentation, support services, sales materials, demonstrations, pilots and related services (together, the “Services”).

The Services are provided by Inferno Analytics LLP, trading as Inferno FMEA (“Inferno”, “Inferno FMEA”, “we”, “us” or “our”).

By accessing or using the Services, you acknowledge and agree to this Disclaimer.

This Disclaimer should be read together with our Terms of Service, Privacy Policy, Cookie Policy and any order form, subscription agreement, pilot agreement, data processing agreement or security schedule that applies to your organisation.

1. Engineering decision-support only

Inferno FMEA is an AI-assisted engineering decision-support platform.

The Services are designed to help professional users draft, structure, review and export FMEA, FMECA, maintenance strategy, reliability, asset management, risk and operational readiness outputs.

The Services do not provide final engineering advice, professional certification, safety certification, statutory approval, asset-owner approval, legal advice, regulatory advice, insurance advice or compliance certification.

Any output generated, suggested, organised, scored, summarised, reviewed or exported through the Services is a draft decision-support output only.

You and your organisation remain fully responsible for reviewing, validating, approving and implementing any output before relying on it.

2. Human review is mandatory

All outputs from Inferno FMEA must be reviewed and approved by competent personnel before use.

Competent personnel may include appropriately qualified engineers, reliability engineers, maintenance specialists, asset management professionals, subject matter experts, asset owners, operations personnel, safety professionals or other suitably experienced people.

Before relying on an output, you must check whether it is:

• technically accurate;
• complete for the asset and operating context;
• suitable for the site;
• aligned with applicable standards;
• aligned with customer requirements;
• aligned with internal procedures;
• legally and contractually appropriate;
• safe to implement;
• approved through the required governance process.

You must not treat any output as final merely because it was generated by the Services.

3. AI-generated outputs may be wrong

The Services may use artificial intelligence, language models, automation, agents, prompts, structured workflows or similar technologies.

AI-assisted outputs may contain:

• errors;
• omissions;
• assumptions;
• hallucinations;
• duplicated content;
• inconsistent reasoning;
• incomplete failure modes;
• incomplete causes or effects;
• incorrect risk ratings;
• unsuitable controls;
• unsuitable maintenance tactics;
• outdated references;
• overgeneralised recommendations;
• wording that appears confident but requires verification.

AI-generated content should be treated as a draft that assists human review, not as a substitute for human expertise.

You are responsible for checking the accuracy, completeness, relevance and suitability of all outputs.

4. No safety-critical blind reliance

You must not rely on the Services as the sole basis for decisions involving:

• life safety;
• public safety;
• environmental harm;
• statutory compliance;
• critical infrastructure;
• hazardous plant;
• pressure systems;
• high-voltage systems;
• confined spaces;
• explosive atmospheres;
• functional safety;
• operational shutdowns;
• production-critical assets;
• emergency response;
• medical, aviation, defence, nuclear or ultra-hazardous activities;
• any other activity where failure may result in death, personal injury, serious property damage, environmental damage, regulatory breach or major operational loss.

The Services may support analysis in high-risk industries, but only as a decision-support tool used under competent professional review.

5. No guarantee of complete risk identification

Inferno FMEA may assist with identifying and structuring failure modes, causes, effects, controls, recommendations and maintenance tactics.

However, we do not guarantee that the Services will identify every:

• asset;
• component;
• failure mode;
• failure cause;
• failure effect;
• hazard;
• control;
• maintenance requirement;
• inspection requirement;
• operational risk;
• safety risk;
• environmental risk;
• compliance requirement;
• commercial risk.

FMEA and FMECA quality depends on the quality of input data, the defined scope, the selected methodology, the operating context, user review, subject matter expert input and final approval.

6. User inputs matter

The quality and usefulness of outputs depends heavily on the information provided by users and customer organisations.

Incomplete, inaccurate, outdated, unclear or misleading inputs may produce incomplete, inaccurate, unsuitable or misleading outputs.

You and your organisation are responsible for:

• defining the study scope;
• confirming asset boundaries;
• confirming operating context;
• providing accurate asset information;
• validating asset hierarchy;
• validating component breakdowns;
• selecting appropriate risk criteria;
• providing relevant site information;
• providing relevant maintenance history;
• checking generated content;
• approving final outputs.

Inferno is not responsible for errors or omissions caused by inaccurate, incomplete or unsuitable inputs.

7. No substitute for site-specific assessment

Outputs must be assessed against the actual site, asset, operating environment and business context.

Generic or AI-assisted recommendations may not account for:

• site conditions;
• asset age;
• asset duty;
• operating envelope;
• process conditions;
• maintenance history;
• regulatory environment;
• local procedures;
• workforce capability;
• spare parts availability;
• shutdown constraints;
• safety controls;
• environmental constraints;
• customer-specific risk appetite;
• insurance requirements;
• contractual requirements.

You must perform appropriate site-specific validation before using or implementing any output.

8. Standards and methodology references

The Services may support, reference or be configured around FMEA, FMECA, IEC 60812-style analysis, AIAG-VDA-style analysis, ISO 55000-style asset management thinking, customer methodologies or other industry practices.

Unless expressly agreed in writing, we do not warrant that:

• an output fully complies with any particular standard;
• a study satisfies every requirement of any standard;
• a study will satisfy an auditor, regulator, insurer, certifier, client or asset owner;
• standard references are complete, current or suitable for your circumstances;
• the Services provide licensed access to any third-party standard.

You and your organisation are responsible for:

• identifying applicable standards;
• obtaining licensed copies of standards where required;
• interpreting standard requirements;
• applying the correct methodology;
• confirming compliance;
• approving final outputs.

References to third-party standards, frameworks or methodologies do not imply endorsement, sponsorship or affiliation unless expressly stated.

9. No professional, legal or regulatory advice

The Services do not provide:

• legal advice;
• regulatory advice;
• safety certification;
• engineering certification;
• financial advice;
• insurance advice;
• procurement advice;
• compliance certification;
• professional sign-off;
• asset-owner approval.

Information provided through the Services is for general business and engineering decision-support purposes only.

You should obtain advice from appropriately qualified professionals before making decisions that require professional, legal, regulatory, safety, engineering, financial or insurance judgement.

10. No warranty of operational outcomes

We do not warrant or guarantee that use of the Services will:

• prevent failures;
• reduce risk to a specific level;
• eliminate hazards;
• improve reliability;
• reduce maintenance cost;
• prevent downtime;
• prevent safety incidents;
• prevent environmental incidents;
• satisfy a regulator;
• satisfy an auditor;
• satisfy an insurer;
• satisfy a customer;
• produce a complete maintenance strategy;
• produce CMMS-ready data without review;
• produce outputs suitable for immediate implementation.

Any benefits depend on the quality of inputs, configuration, review, implementation, governance and ongoing maintenance by the customer organisation.

11. Reports, exports and load sheets

Reports, exports, spreadsheets, dashboards, summaries, decision sheets and load sheets generated through the Services are draft outputs unless formally reviewed and approved by your organisation.

Before using exports in any operational system, including a CMMS, EAM, ERP, document management system or asset information system, you must check:

• data accuracy;
• field mapping;
• coding requirements;
• asset hierarchy;
• task descriptions;
• frequencies;
• risk ratings;
• approval status;
• version control;
• system compatibility;
• governance requirements.

Inferno is not responsible for incorrect loading, configuration, interpretation or implementation of exported data.

12. Customer approval remains required

Inferno FMEA does not approve, authorise or implement customer decisions.

Your organisation remains responsible for:

• governance;
• review;
• approval;
• change management;
• implementation;
• training;
• communication;
• verification;
• monitoring;
• audit;
• record keeping.

No output should be implemented until it has passed the customer’s required review and approval process.

13. Third-party data and integrations

The Services may rely on, link to or integrate with third-party systems, providers, models, APIs, documents, standards, cloud services, payment providers, analytics tools or other services.

We are not responsible for third-party services, third-party content, third-party systems, third-party outages or third-party terms except to the extent required by law or expressly agreed in writing.

Third-party services may change, become unavailable, produce errors or affect the performance of the Services.

14. Product development and beta features

The Services may include early-access, beta, preview, experimental or newly released features.

Such features may be incomplete, unstable, unavailable, changed or withdrawn at any time.

You should not rely on beta, preview or experimental features for production, safety-critical or operational decisions unless expressly agreed in writing and reviewed by competent personnel.

15. Demonstrations, pilots and examples

Demonstrations, pilots, sample studies, screenshots, templates, marketing materials and example outputs are provided for illustrative purposes only.

They may not reflect:

• your assets;
• your site conditions;
• your risk criteria;
• your operating context;
• your standards requirements;
• your data quality;
• your final output quality;
• actual product performance in every environment.

Any estimated time saving, cost saving, productivity improvement or quality improvement is indicative only and not guaranteed.

16. Availability and performance

We aim to provide a reliable service, but we do not guarantee that the Services will always be available, uninterrupted, secure, error-free or free from delays.

Availability may be affected by:

• maintenance;
• updates;
• outages;
• internet issues;
• third-party provider failures;
• cloud service interruptions;
• AI provider issues;
• security incidents;
• customer network restrictions;
• browser or device limitations;
• events beyond our reasonable control.

Where service levels are required, they must be agreed in writing in an order form, enterprise agreement or service level agreement.

17. Cybersecurity limitations

We take reasonable steps to protect the Services, but no system is completely secure.

We do not guarantee that the Services, customer systems, networks, integrations, files or communications will be immune from unauthorised access, cyberattack, malware, data loss or security incidents.

You and your organisation are responsible for maintaining appropriate internal security controls, including secure access management, account administration, endpoint security, staff training and review of user permissions.

18. Limitation of liability

To the maximum extent permitted by law, Inferno is not liable for loss, damage, cost or expense arising from:

• use of or reliance on draft outputs without competent review;
• incorrect, incomplete or unsuitable user inputs;
• customer implementation decisions;
• engineering decisions;
• maintenance decisions;
• operational decisions;
• safety decisions;
• asset management decisions;
• regulatory decisions;
• use of outputs outside their intended purpose;
• failure to obtain professional review;
• third-party services;
• unauthorised access caused by customer-side security failures;
• data loading or migration errors;
• production loss;
• asset failure;
• downtime;
• safety incidents;
• environmental incidents;
• regulatory penalties;
• business interruption;
• loss of profit;
• loss of opportunity;
• indirect or consequential loss.

Any liability we have is limited as set out in our Terms of Service or any applicable written agreement.

19. Non-excludable rights

Nothing in this Disclaimer excludes, restricts or modifies any right, guarantee, condition, warranty or remedy that cannot lawfully be excluded, restricted or modified.

Where any law gives you rights that cannot be excluded, this Disclaimer applies only to the maximum extent permitted by that law.

20. Relationship with other agreements

If there is a conflict between this Disclaimer and a signed enterprise agreement, master services agreement, order form, data processing agreement or security schedule between Inferno and a customer, the signed agreement will prevail to the extent of the conflict.

If there is a conflict between this Disclaimer and the Terms of Service, the Terms of Service will prevail to the extent of the conflict.

21. Changes to this Disclaimer

We may update this Disclaimer from time to time.

When we make changes, we will update the “Last updated” date at the top of this page.

If changes are material, we may provide additional notice, such as by email, in-app notice or website notice.

Your continued use of the Services after the updated Disclaimer takes effect means you acknowledge the updated Disclaimer.
